Shows name, age, gender, marital status, date of birth and place of birth of patient; date admitted; prior address; condition on admission; and date of discharge or death.  The Infirmary register prior to 1867 has been lost as reported in the 1883 History of Union County, Ohio.  Certain portions of the records are restricted pursuant to ORC 149.43 due to the fact they contain medical information.
