Wellness Program Information

  • Health & Benefits Fair
    • When? Wednesday, October 24, 2018 from 7am - 10am
    • Where? 940 London Avenue (Conference Rooms A & B), Marysville, Ohio
    • Who? Employees and spouses are invited!
    • Why? Visit with benefit providers and wellness vendors, and get your flu shot (optional and FREE with health insurance)! Earn up to 75 wellness points - 25 for attending and another 50 for your PREVENTIVE CARE flu shot. 
  • Memorial Hospital Wellness Center Class Schedule
    • When? Classes are 1 hour in length and are offered through December 2018.
    • Where? Memorial Hospital's Wellness Center located at 500 London Avenue, Marysville, Ohio
    • Who? Classes are FREE and open to the public!  No registrations are needed, though you are encouraged to call to verify dates and times. 
    • Why? Hear directly from health experts in our community on a variety of wellness topics. Attendance is considered a County/Local activity. Earn 25 wellness points for each class (up to the maximum points/program year).

The 2019 Wellness Program runs from October 15, 2018 through August 15, 2019. There is a look-back period to October 1, 2018, so activities will count between October 1, 2018 and August 15, 2019. Activities may be self-reported in your Interactive Health portal beginning October 15, 2019. All employees and spouses are invited to actively participate in the County’s wellness program and engage in healthy lifestyle programming activities and events.

To complete the 2019 Wellness Program and earn the wellness incentive (i.e., premium discount) in 2020, employees and  spouses enrolled in the CEBCO medical plan must earn a total of 600 wellness points, which consists of one (1) required activity worth 300 points - Health Evaluation - which includes a health questionnaire and biometric screening) and a combination of elective activities. Non-insured employees may also earn the wellness incentive for 2020 by earning a total of 300 points. These employees must create an Interactive Health portal and self-report elective activities to qualify for the wellness incentive in 2020. Non-insured employees are not eligible to participate in the Health Evaluation that is offered through Interactive Health. 

Individual Portal Accounts: Employees and any spouse covered on the County’s health plan will create, manage and track wellness points through individual portal accounts at www.myinteractivehealth.com. First time users will need to use the following sponsor code to access the program: TMUYW.

Shortened Program Year: The 2019 CEBCO Wellness Program will end on August 15, 2019. All portions of the program must be satisfied by this date to earn the wellness incentive for the 2020 calendar year. After the 2019 program year, we will resume a 12-month wellness program period. 

2019 Activities/Points Restructuring: CEBCO has made some slight changes to certain wellness activities and points that may be earned. A consolidated Personal Health Score will now be calculated based on Health Evaluation results, instead of members earning 50 points (each) for in-range blood pressure, cholesterol, and glucose results. The Personal Health Score will be based on five controllable health factors: blood pressure, LDL cholesterol, glucose, triglycerides, and tobacco use. There will be three score categories:

  • Low Risk = -20 to 0 (150 points will be earned for this score)
  • Moderate Risk = 1 to 25 points (75 points will be earned for this score)
  • High Risk = 26+ points (0 points will be earned for this score

The Union County Wellness Committee will continue to host wellness events and activities throughout the year, including a Health & Benefits Fair on October 25, 2017, onsite health screenings in early 2018, Lunch and Learns, a summer Wellness Fair, and MORE!

Elections made during the most recent Open Enrollment period will remain in effect through the upcoming calendar year, unless there is a Qualifying Life Status Change that may impact your benefit eligibility and/or elections. Please contact HR dor the Auditor's Office if you have a qualifying change during the 2020 benefit year, as any applicable changes are time sensitive and must be made within 30 days of the qualifying event. 

Summary of 2020 Insurance Benefits

include but are not limited to: copies of marriage certificates and tax records for spousal coverage; birth certificates, adoption or other legal records for dependent children. *

  • Cafeteria PlanA Cafeteria Plan is a reimbursement plan governed by IRS Section 125 which allows employees to contribute a certain amount of their gross income to a designated account or accounts before taxes are calculated. Union County offers these accounts for certain insurance premiums, medical expenses and dependent daycare expenses, from which eligible employees can be reimbursed throughout the plan year as they incur the expenses.
    • To learn more about these benefits, click on this link to view the Summary Plan Description. Annual elections must be made to participate in the medical and dependent care reimbursement accounts, and annual re-enrollments are required.
    • Current participants may submit claims to MyCafeteria Plan (Union County's cafeteria plan administrator) one of four ways, to get reimbursed for eligible expenses:
      • Download the "myCafeteriaPlan On-The-Go-App" from the App store, enter passcode, select File a Claim, and follow prompts.
      • Online (www.mycafeteriaplan.com): Log into your account and click the "File a Claim" link on the left-hand menu.
      • Fax: The Claim Form may be faxed to (937) 865-6502
      • Mail: The Claim Form may be mailed to myCafeteriaPlan, 432 East Pearl St., Miamisburg, OH 45342
  • Supplemental Benefits: Additionally, Union County provides full-time employees with the opportunity to enroll in voluntary, supplemental benefits through the convenience of payroll deductions. Voluntary benefits cover a variety of areas that you, as an employee, can pick and choose to meet your individual needs. Possible options include, but are not limited to: Short-Term Disability, Cancer Care Plans, and supplemental life and vision insurance offerings. Premiums for these voluntary plans are 100% employee paid. Enrollment is time sensitive for new employees.
